SAVE THE DATE – Friday April 27, 9:30 to 2:30 – for a wonderful opportunity to learn about community programs and meet the dedicated leaders of many of the North Sound Region’s care coordination resources. The North Sound P-TCPI Regional Leadership Team is offering a Care Coordination Summit on Friday April 27, 2018. Invited speakers include leaders from Parent to Parent, Help me Grow, North Sound 211, Seattle Children’s Center for Children with Special Needs, Whatcom Taking Action/SEAS line, Hearing Speech and Deafness Center, and our North Sound ACH Pathways Program. There will be written information on each program and time for networking over lunch. This is a great opportunity for networking, learning about helpful tools to connect families to a variety of useful resources, and for learning about Washington’s Pediatric-Transforming Clinical Practice Initiative. This day is geared towards medical and behavioral health clinic staff, including front and back office, medical directors, nurses, referral coordinators, providers and clinicians. This free Summit will be held at the Skagit PUD Aqua Room at 1415 Freeway Drive, Mt Vernon, from 9:30 am to 2:30 pm. Lunch has been graciously sponsored by our P-TCPI project partner, Molina Health Care.
